Contributing partners

Ontario Health and the Ontario Ministry of Health work a number of implementation support partners and a number of other health-system partners to support Ontario Health Teams (OHTs) and their Primary-Care Networks (PCNs).

Events hosted by Ontario Health, the ministry and these contributing partners are a key part of the support being provided to OHTs and PCNs.
